While everyone in primary care are focused on the Triple Aim – reducing per capita cost, improving the patient experience and improving population health - some practices have added a fourth goal: Improving the clinician’s experience.  This presentation will focus on several new initiatives focused on all four goals all under the umbrella of a Patient-Centered Medical Home.  This will include new models for team-based primary care and integrated care for complex patients.
